An NEA specialists’ meeting “Identifying and Overcoming Barriers to Effective Consideration of Human and Organisational Factors in Event Analysis and Root Cause Analysis” was held in Paris, France on 21-22 September 2009. A total of 17 participants from 10 countries representing licensee organisations, regulators, international organisations and an independent consultant attended the meeting. The participants identified barriers to the effective treatment of human and organisational factors in root cause analysis and made recommendations to mitigate the effects of these barriers.
